OSHA Form 300 Summary
Worksite employers with 10 or more employees are subject to the recordkeeping requirements of the federal Occupational Safety and Health Act and are reminded by our HR department to post the OSHA Form 300A, Summary of Work-Related Injuries and Illnesses, from February 1 – April 30. (every year)
Which Forms are involved?
Record Keeping Requirements
- Employers with more than 10 employees are required to keep a record of serious work-related injuries and illnesses. (Certain low-risk industries are exempted.)
- Some minor injuries requiring first aid only do not need to be recorded.
OSHA 300 Form Summaries
The following forms/reports are accessible from within the AdvanStaff HR Manager Portal.
Form | Description |
300A | Summary of Work-Related Injuries and Illnesses. * Required to be posted in a visible place (for example, by your employment poster) on an annual basis. * Post this summary page from February 1 to April 30 of the year following the year covered by the form |
300 | A log of work-related injuries and illnesses. * Required to be kept on site in a secure place in case of audit. |
301 | The Injury and Illnesses Incident Report. * A report form used to provide details about an incident. |

NOTE: Worksite managers are responsible for adding any reportable incidents & hours that were not administered through/by AdvanStaff HR during the reporting period and that are not included in the log & summary.
If AdvanStaff HR supplied your form and items are missing, that means AdvanStaff HR is unaware of the occurrence. Please contact our offices immediately.
Maintaining and Posting Records
- The records must be maintained at the worksite for at least five years. Each February – April, employers must post a summary of the injuries and illnesses recorded the previous year.
- Upon request, copies of the records must be provided to current and former employees, or their representatives.
Electronic Submission of Records
- AdvanStaff HR handles the record keeping and provides the postings if we are the assigned agent covering all aspects of the workers compensation program.
- Any worksite employer not utilizing AdvanStaff HR Risk Services for workers compensation policy management is required to electronically submit the summary of injuries and illnesses to OSHA directly.
