Where can I obtain a copy of my HPN/SHL Medical ID card?

Before you can determine which carrier to contact to request a replacement card, you will need to confirm your benefits. To do this:

  1. Login to the Employee Self-Service Portal (ESS)
  2. Click on “Benefits”
  3. Click on “Benefits Summary”
  4. Confirm your plan

Then you are ready to request a replacement card from the carrier.

PDF versions of Medical ID cards for Sierra Health and Life & Health Plan of Nevada are easily obtained through the @YourService web portal.

This direct link will take you to the registration portal to create an account to access your card(s) 24 hours a day, 7 days a week.

Remember, this is ONLY for the Medical ID card option through Health Plan of Nevada (HMO) or Sierra Health and Life (PPO).

Dental and Vision coverages are not represented through this portal and can be found in another article. 🙂
